All of our required notices and plan document information can be found on our benefits main page under the "Required Notices" section.
Specific Premera plan documents can be found directly in after you sign in by clicking on...
- "Benefits" in the toolbar at the top right →
- "Benefit Details" in the drop-down menu →
- "If you don't find the information you're looking for, check your benefit booklets" at the very top of the page under the "Benefit Details" header.
Following the steps above will allow you to download the Premera plan booklet specific to your medical and/or dental plan for reference.
